Personal Health Record (PHR)
Personal Health Record (PHR) is an online application which provides patients with access to their maternity record. PHR contains information (relating to the patient's current pregnancy) that they have given to their care provider/midwife during first booking and other hospital or community appointments.
Patients can enter information into PHR. Some information will be sent to their hospital record so their midwife can see it. Some pages are intended for private notes and will not be displayed in the hospital record. PHR also has links to useful information and advice relating to pregnancy and birth.

PHR opens at the Home page. There are cards that you can click or tap to open different pages. Alternatively click the Menu button to open the side menu.

Pregnancy is divided into three trimesters, each lasting for approximately three months. Any links on this page are to external websites which are not part of the PHR application. To view information which is relevant during your current trimester:
Click the Trimester card on the Home page.
- If gestation is between 0 and 12 weeks, the First Trimester page opens.
- If gestation is between 13 and 27 weeks, the Second Trimester page opens.
- If gestation is 28 weeks and above, the Third Trimester page opens.
Links are displayed in categories. For example, Keeping Well in Pregnancy and Labour and Birth.

Recommendations contains personalised advice based on the patients own health, medication, or circumstances. To view recommendations:

Care Plans are the preferences and goals you have discussed with your midwife. They can relate to care before (antenatal), during (intrapartum), and after (postnatal) the birth.
Care plans are read-only. To discuss answers to any questions or add new information, contact your midwife. If any of your personal information is incorrect, use your Data Quality Contact details to notify your hospital.
To view your care plans click the View My Care Plans card on the Home page.

Contains contact details for data quality, useful addresses, and hospital departments.
Click Menu to open the side pane and click Contact Information.

Pre Booking information is a combination of medical history and current circumstance. To view or edit your pre booking forms:
- Click the Complete My Pre Booking card on the Home page.
- Click a card to open a questionnaire e.g. Medical History.
- Click tickboxes to answer questions. A questionnaire can be saved without every answer populated. If unsure on how to answer any questions, ask your midwife to help you complete the questionnaire.
- Click Submit when finished.

Birth preferences are used to record any specific criteria regarding the where who and how of a pregnancy. To view or edit your birth preferences:
- Click the Complete My Birth Preferences card on the Home page.
- Click a card to open a questionnaire.
- Click tickboxes to answer questions. A questionnaire can be saved without every answer populated. If unsure on how to answer any questions, ask your midwife to help you complete the questionnaire.
- Click the Submit button when you are finished.

Observations are used to keep a record of weight, blood pressure and urine test results. PHR shows the data in a graph to show result changes throughout a pregnancy. Observation information is for personal use only, if you have any concerns, please contact your midwife directly.
To add a new observation:

To add a new journal entry:
Verify patient email address

Once authorised, the patient must do the following to access their EuroKing Personal Health Record:
- Click the Verify Email Address link in the email. The link is valid for 72 hours. If a patient informs you that their link has expired, deauthorise then reauthorise. This will trigger a new email.
- Enter your date of birth and password. Click the Register button.
- Enter your email address and new password and click Login.
- Enter a nickname (the name you would like your midwife to use) and enter your height in metres. (For example 5 feet 6 inches = 1.68 metres).
- Click Save.
